This half day training offers Early On^® providers the information and resources in order to support families and children as they transition from Early On^®. The presentation clarifies the 2011 Part C Regulations and the required steps of transition. The training highlights the following:

• The key changes in the transition process
• Notification
• Transition Conference
• Transition Plan
• Timelines and documentation
• Options for placement
• Roles and responsibilities
• Procedural safeguards related to transition
• Child and Family Outcomes related to transition

This training is designed to provide a forum for discussion and problem solving.  The training focus is to apply knowledge, discuss challenges, and exchange effective strategies related to the transition process.
